Home and Community Based Services (HCBS Waiver)/Medicaid Service Coordination
HCBS Waiver services are supports for individuals with developmental disabilities who qualify for ICF level of care but choose to live at home or in a smaller certified residence. These services assist individuals to become as independent as possible and help the them develop a healthy lifestyle of their choosing. HCBS Waiver services can be delivered at home or in the community. They can occur in small groups or one-on-one. Services available include At Home Residential Habilitation, Day Habilitation, Respite, Medicaid Service Coordination, Assistive Technologies, and Environmental Modifications.
